Default Dog bone bushings

I have the Dog bone MM's and The upper one is giving me trouble. The bushing is prying itself away from the mount. The mount is torqued to 80ftlbs with blue locktite and a washer also. This isn't the first time its happened. I removed it and pushed the bushing back in re torqued it and after going out for a pull it looks like this again. Not sure what to do here... Its affecting the movement of the motor. Its less than 1 year old and i purchased them new from modern performance Any ideas?

If you put a large diameter washer (fender washer) next to the hex head of the bolt, all will be good.

It already has one there, the mounts came with washers. The washer is the same diameter as the mount. Thanks for the idea though.

did you spin the top silver piece around 180* before installing? If not that is your problem, read the install directions in the sticky /\ above

I am not seeing how it is even possible for the bushing on the inside to come out like that if the bolt is torqued down and the washer is in place. Are you sure you did not reverse the washers and put the one for the lower mount on the upper mount?

Ok so i took it off today, and it was flipped the correct way. Here is what the mount looks like off the car and all bolted together before going on the car. The bushing looks together when its off the car, a little gap but barely. See anything I'm missing? I just don't want to bolt it up and have it do the same thing again.

Just an update. I got the new mount yesterday, buttoned up everything with 12.9 grade bolts and loctite. Took it out for a drive and did some pulls, car felt great. Came home and the mount hadn't moved! So thanks again AGP for the excellent customer service, concerning the old mount.
