Default Warped disc?

What are the odds that my disc is warped? I have the FX300 with about 700 miles on it. I replaced the pivot ball and TOB fork at the same time. Car drove fine first 4 days then became hard to shift. Made the clutch pedal adjustment and it drove fine again for about 3 1/2 weeks. It started to get hard to shift and after 2 days it won't even go into gear anymore, made adjustments again, bushings are fine on the cables. Replaced the master and slave cylinders today and it didn't do anything. Car goes into gear fine when the engine is off. If I put it in gear and try to start it, it won't turn over because the clutch is engaged even with my foot the entire why to the floor.

Any ideas what my problem could be? I am getting it towed to a garage next weekend to drop the tranny and inspect it. Figured I would ask you guys and see if you had any ideas?

That is what I'm starting the think too. Sucks, I'm not a fan of dropping the tranny again, but it must be done.

Bump for some CM's help, what do you guys think? My clutch was installed by a Dodge mechanic 700 miles ago, so I know it was done correctly and I know I broke it in correctly too. Any ideas why it won't disengage?
