Default Deep "scoring" in rotor???

What would cause a 1/64 (d) x 1/8"(w) score in my front rotor? That's a rough est, I can definitely feel it with my fingernail. The pads are all but new, I have a good 3/8" left on them. I just noticed it this morning. I'm running stock rotors and mild Hawk pads. Any ideas?

They are Hawk "HP Plus" pads and see photos below for visual description.

the HP+ pads are fairly aggressive, especially when cold. That looks like something got stuck between the pad and the rotor. Is the pad surface perfectly smooth?

Yea that was my assumption as well. Just was not sure if that was possible. I will have to check the pads later this week.

pads have a split down the middle right....prob got a rock in the split ...imagine the luck
