I wouldn't say I have a review...but I do get an evaluation one a year.
What does that consist of? I posted about this last year as well...but I'm always curious to find out what other people go through. I originally got my self review sheet back in December...so I filled that out (took me about a month, lots and lots of writing)....and them my manager reviewed me and wrote his own notes...then HIS manager reviewed both of our reviews and added stuff...THEEEEN...it's passed up to HR who reviews everything, makes note of any problems or potential 'red flags', then the review is passed back and eventually you meet with your manager and manager's manager for 30min to discuss the review and find out if you get a bonus and/or raise.
*breathes*
Pretty crazy, but it's good because if you get a good review, everything is well documented and known.
I say, "Hey I had a pretty good year. I should give myself some more money."

we have annual reviews at Intel, same procedure basically with the manager and hr reviewing the document, but it starts out by cranking out whats called a brag sheet that I write, in essence letting my manager know what I did all year and if I write it well enough the result should be a raise and stock options(which are pretty worthless by the way) we get bonuses 3x a year based on the companies performance and thankfully not on the individual review.
