My transmission with about 67k miles in it had an abundance of ware on the fork:
Notice the nice grooves on the new one by S&P. My old unit didn't have any. This was causing the TOB to be engaged unevenly, and it even ended up breaking one of the tabs off the TOB that I found rolling around at the bottom of the bellhousing when I did my clutch job. My pivot ball also just fell right out upon removing my clutch release fork. Doh! Good thing S&P sends you one with their forks!

Transmission must be taken out, clutch must be removed and then you can get to the fork. The fork is what the TOB sits on and slides in when the slave cylinder is engaged using the clutch.
I can say changing my clutch release fork made a world of difference in my shifts. Very smooth now, and no more 1st and 3rd gear bitch-fests trying to get it into gear.
