Anyone ever have their tach adapter go bad?

My safc reads my revs just fine at idle.

But when I open the throttle and the revs climb, the safc reads all over the place, sometimes 0, sometimes a few thousand rpm's off.

My tach signal is good all the way to the tach adapter. I checked the voltage and I checked what the PCM is reading with an OBD tool and the rpm's are perfect with the obd. I also checked all grounding wires.

What do you guys think?
